REACH Care Coordinator (1 Bilingual, English/Spanish)

PRIMARY FUNCTION: 
Responsible for coordination of services for children with severe emotional problems and their families.
PRIMARY DUTIES: 

CLIENT SUPPORT 1. Assemble child and family team through family interviews identifying family members, natural supports, agency representatives and other persons significant in the child and family’s life. 2. Assess family’s strengths and needs. 3. Coordinate meeting of child and family team and develop an individualized Plan of Care (POC), including a comprehensive crisis plan. 4. Advocate for the child and family when new services or resources need to be developed. 5. Provide or secure support and crisis services to families as needed. This may be done through face-to-face contact, phone contact or availability by beeper or an on-call system. 6. Serve as a liaison between the child/family and community service providers and assure acquisition of services. 7. Provide transportation to children and families, if needed for purpose of medical appointments, crisis/respite resources, etc. 8. Monitor provision of services through child and family team, modify the POC as needed and SAR when new services are added and/or existing services discontinued. MONITORING, REPORTING AND COMPLIANCE 9. Submit POC and Service Authorization Request (SAR) to WM, including services needed from other providers and associated costs. 10. Prepare reports, correspondence or other information required by the Court; Collaborate with the Bureau of Milwaukee Child Welfare and probation workers. 11. Participate in Quality Assurance/Quality Improvement studies. 12. Complete require measurement instruments to evaluate the effectiveness of services provided. GENERAL 13. Attend all trainings and meetings as directed 14. Other duties as assigned related to the overall purpose and needs of the program.

QUALIFICATIONS: 

1. Bachelor Degree in Social Work, Psychology, Nursing, Sociology, Occupational Therapy or related field. 2. Experience in human services, preferably case management. 3. Able to demonstrate the ability to work and communicate frankly and effectively with families to implement problem solving strategies, understand issues, and to make effective connections for services. 4. Demonstrate cultural competency with diverse groups and ability to work with individuals from diverse economic backgrounds in respectful manner. 5. Excellent communication skills, both oral and written, interpersonal relationship skills, and able to work effectively with others from culturally diverse, linguistic and economic background; both in a team and individual environment. 6. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. 7. Highly developed organizational skills and able to manage multiple priorities. 8. Knowledge and experience with experience in using Microsoft Office suite. 9. Transportation, valid Wisconsin Driver’s license, state minimum auto insurance and must meet La Causa, Inc. driving standards. 10. Able to work flexible hours, including evenings and weekends depending on program needs. 11. Must successfully complete background checks.
