Recovery Support Case Manager

PRIMARY FUNCTION: 
The Recovery Support Case Manager provides assistance to individuals referred by the WIser Choice program who are in need of regular contact to remain engaged in treatment, but who are not in need of services by a Recovery Support Coordinator. The Recovery Support Case Manger provides continuity of care and follows the client throughout their course of treatment and as the client moves through different levels of care and various RSS Providers until these services are no longer necessary and/or appropriate.
PRIMARY DUTIES: 

INITIAL CONTACT AND PLAN DEVELOPMENT 1. Make contact with a referred client within 24 hours of notification by the Central Intake Unit (CIU) 2. Make every reasonable effort to locate clients who have been referred by Central Intake for Case Management Services, but who do not keep their initial or subsequent appointments with the Recovery Support Case Manager or other identified provider CONTINUING CLIENT SUPPORT 3. Provide light case management services as required, including, but not limited to: coordination of medical appointments, coordination of social services, coordination of housing provisions, coordination of legal advocacy, benefits advocacy, troubleshooting and intervention to ensure individuals get to treatment and other necessary appointments and coordination of transportation needs 4. Provide 24-hour, 7-day-a-week crisis access to assigned clients 5. Provide light care coordination services to each client throughout the duration of enrolment in BHD AODA Services 6. Refer individuals with less intensive needs to Recovery Check-Up services 7. Make contact with each client on at least a weekly basis MONITORING, REPORTING AND COMPLIANCE 8. Routinely update client contact information and/or the “Client Locator Form” 9. Monitor to ensure that service authorizations are current. 10. Based upon information obtained from the RSS Tool, request extensions of and additional service authorizations from BHD as indicated by up-to-date patient placement criteria provided by the clinical treatment provider 11. Monitor the provision of services by agencies as authorized 12. Document in the client record, according to BHD protocol, all services provided 13. Complete, in face-to-face interviews, the required outcome data collection instruments for each client at intake when needed (i.e. for particular sub-groups for which the CIU does not perform this function), again at 6 months post-intake if the client is still in service, and within 2 weeks of discharge from all clinical services. 14. Assure compliance with program completion measurement protocols for each client 15. Administer a client satisfaction instrument according to a protocol to be designed by BHD GENERAL 16. Attend all trainings and meetings as mandated by BHD and/or as directed 17. Other duties as assigned related to the overall purpose and needs of the program. NE

QUALIFICATIONS: 

1. Bachelor Degree in Social Work, Psychology, Nursing or related field. 2. Experience in human services, preferably case management. 3. Able to demonstrate the ability to work and communicate frankly and effectively with families to implement problem solving strategies, understand issues, and to make effective connections for services. 4. Demonstrate cultural competency with diverse groups and ability to work with individuals from diverse economic backgrounds in respectful manner. 5. Excellent communication skills, both oral and written, interpersonal relationship skills, and able to work effectively with others from culturally diverse, linguistic and economic background; both in a team and individual environment. 6. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. 7. Highly developed organizational skills and able to manage multiple priorities. 8. Knowledge and experience with experience in using Microsoft Office suite. 9. Transportation, valid Wisconsin Driver’s license, state minimum auto insurance and must meet La Causa, Inc. driving standards. 10. Able to work flexible hours, including evenings and weekends depending on program needs. 11. Must successfully complete background checks

NORMAL WORKING CONDITIONS: 

Work is generally performed in office and travel to client homes and for appointments. Frequent walking, standing, hearing, seeing, writing/typing required. May be exposed to infectious diseases. Local travel required; some state-wide travel as needed. Flexible hours required; as dictated by program needs.
