PRIMARY FUNCTION:
Responsible for providing support to the Coordinators and for coordination of services for children with severe emotional problems and their families.
Primary Duties

- Assist Program Supervisor (provide support, problem resolution and feedback to care coordinators, help plan and conduct staff team meetings, mentor new staff, assist care coordinators in developing and maintaining formal and informal resources, etc.). E
- Train new and current staff (program policies and procedures, Wraparound processes, etc.). E
- Review documentation (progress notes, plans of care, transitional/disenrollment plans, community safety plans for clients, etc.). E
- Facilitate all aspects of services for participants, assemble child and family teams, manage assigned care coordination cases (transport participants as needed, assemble child and family team, assess needs and strengths, develop an individualized Plan of Care (POC), advocate, provide or secure services, testify in Court, etc.). E
- Serve as a liaison between the child/family and community service providers and assure acquisition of services. E
- Monitor and negotiate provision of services, and agreements, through child and family team, modify the POC as needed and SAR when new services are added and/or existing services discontinued. E
- Complete/collect/process and/or submit required items by due dates. E
- Follow legal, organizational and contractual requirements, laws and policies. E
- Establish and maintain good communication, collaboration, and cooperation with all stakeholders. E
- Attend meetings and professional development activities as requested. NE
- Follow all duties of a mandated reporter. E
- Perform other related duties as assigned. NE

Required Qualifications
- Bachelor Degree from an accredited school in Social Work or related field (i.e.; psychology, sociology, counseling).
- One (1) year of Wraparound care coordination experience.
- Able to work and communicate frankly and effectively with adults, families, and all team members to implement problem solving strategies, understand issues, and to provide crisis services in an ethical, respectful, responsible manner.
- Strong communication skills, critical thinking, cultural competency, and interpersonal relationship skills.
- Highly developed organizational skills and able to manage multiple priorities.
- Knowledge and experience with experience in using Microsoft Office suite.
- Transportation, valid Wisconsin Driver’s license, state minimum auto insurance and must meet La Causa, Inc. driving standards.
- Able to work flexible hours, including evenings and weekends depending on needs of the youth/family and program needs.
- Must successfully complete and pass all background checks.

WORKING CONDITIONS:
Work is generally performed in field and office environments. Frequent travel; infrequent state-wide travel. Flexible hours required; as dictated by program needs.
